1) Refering to section 6.0, for DUN in Hulu Selangor which was won by MCA, who should we submit the form to? We understand that we can also submit to Pusat Khidmat Rakyat DUN, but the centre is no longer operate, please advice what should we do?
2) If the forms are submited to MP or Ahli Majlis, can their stamp be recognise on section 6.0?
3) According to Ooi Leng Han, we must use the original form with Serial Number instead of photocopy, is this true?
4) For convenient purpose, can Ketua Kampung or Pengerusi JKKK sign on behalf of DUN on section 6.0?

An newspaper article mentioned that this forms has to be pre-numbered. Is there any form being superseded ? Please advise when and where can the latest version documents to be downloaded. Thanks. HB

if you can, please come to my office or go to any office to collect the new form with serial number.
